Ah, I think I see the problem, as its not a campaign it wont appear in the campaign menus.
Play Survival > Play with Friends > Create new lobby > map > addon campaign.
Set lobby to private/local server and should work just fine in single player.

What do you mean "single mod"? It works in single player since I last tested it.
