I agree 'free hospital' doesn't make sense, hospitals that cost half the production to build and/or granted additional health would make more sense, maybe even reduced maintenance for it. Obamacare didn't magically make all healthcare cheaper but it drastically improved the quality of policies and the proportion of people covered, which in terms of economics is a more efficient and valuable outcome - a more difficult concept than 'cheaper' but true nonetheless. Unionisation is a bit of a strange one too, unionism has been carefully murdered in most of the US economy, collective bargaining is the ultimate counter-weight to corporate power and has, intentionally, been neutered for that very reason - I can't think of anything in particular Obama did to counter this trend.

@ZKP Unionized workers are less productive and Obama Care increased the cost of healthcare to Americans, an actual translation would be higher hospitcal maintenance and higher worker maintenance.

Yeah because unionized workers and free hospitals aren't good game translations of Obama's actual policies in context to civ 5's game mechanics. Your antagonism is showing. This is a game, keep your personal politics out of it and argue on the basis of the actual game mechanics and what makes sense in that context. I'm still looking for a good Donald Trump mod that isn't Alt-Right influenced as well.
