Having played Vermintide 1, I can say this is pretty much what I expected from a sequel. Not much has changed really, but I don't really call that a bad thing. At its very core, the gameplay is still satisfying and fun. I find it more fun than Left 4 Dead to be honest, simply due to the fact its much more in depth than a simple click and shoot. I mostly play on Legend which I find a nice challenge and I find that teamwork works best at Legend than on any other difficulty (minus a few lone wolf players who don't listen). As for the DLC, I bought all of them except Winds Of Magic which I have no intent on buying. The other DLC's are worth it in my opinion. More maps, pretty good design across all of them and of course Horn of Magnus from VT1. Winds of magic does not appeal to me and if anything just splits the player base and is relatively pointless so I would say avoid that. The loot grind itself is of course RNG based but naturally picking up grims and tomes enhance your chances and to be honest, loot itself isn't overly important. No real need for reds, nice to have but you can still easily complete legend without them. The game is just a fun mindless, rat killing game which doesn't really get boring.
