How do you place elevators? I placed one on the ground floor but how do I place it on the second floor so they connect

I just gave up trying to figure out how to start. I can't place a door so no entrance. I tried restarting several times but the tutorial is worthless. I'll go watch Colonel Failure try to play it. Maybe he figured out how to place a door.
