You should make a separate crawl mod that activates when you break both ancles/legs.

i think that he is using the base game animation for the crawling

That would require per-weapon animations and code, which would be a huge hassle and would have major compatibility issues, great idea though.

An optional settig:
Add a chance to get minor injuries when crawling, like scratches.

It would be nice passing through more than 1 car at the time, or making the animation faster when panicked
