Almost 100% sure that your friend just doesn't have it installed. The mod uses no custom assets besides the sound effects so it will look like it's working for your friend even if he doesn't have it installed.
Try installing "Addon share" to your game https://steamproxy.com/sharedfiles/filedetails/?id=973145750 , and it should automatically download the files for him when he joins your game.

player that has joined my local server can not hear notes that I play, nor that he plays, but I hear the notes no matter who plays, is mp an issue with this mod?

Maybe, you could make an addon, for, not machines that look like this, but an addon that puts Clarinets (Requested: Bass Clarinet, Soprano Clarinet, Contrabass Clarinet) into the game, that are animated AND make sounds AND you can play?
