It's a matter of RNG if you will encounter one, but ESE seems to increase the pool of items for sale as well as various shopping limit behavior for NPCs.
Without some mod to increase the size of NPC stock variety-- you will rarely come across it and will just have to sit down and wait for refresh or find another clothing shop. I did two playthroughs prior to this one and I never seen the Mekakushi in them-- so it's significantly rare. Unsure if that's intentional or not.
