Getting katana block in 30 ms seems impossible. Even with the help of auto-clickers you'll most likely fail to get it (I tried and got 66,49,33 ms only). Actually just spamming left-click (naturally the fastest button) can get these near values. But not 30ms or less.
But I did get it eventually. It happened during a single player trials, particularly ONI BATTLE on some later stage (purple or brown belt). A single oni swung on my left, I started spamming left-click attack. All of a sudden another oni spawned nearby and swung on my left too. Next - I got the reaction of 10ms and the achievement.
Now my theory.
The game lets you block an oni attack only after he swings his sword fully, and it all messes up the reaction times (the mouse is locked, then it's unlocked but probably after 30ms have already passed). But when two onis attack you nearly at the same time (it happens rarely), your mouse is already unlocked when the first oni tries to strike you, and the click and the reaction time are registered for the second oni. This scenario seems quite hard to perform, as oni attacks seem random.
Alternatively, it might as well be that the fastest reaction times can be registered only on the fastest attacking onis, which are also rare.
So, in the end my advice. Just get to some harder stages of the game and continuously play ONI BATTLE just spamming left-click attack.

Ultimate ninja

The description says you need to get straight As on every belt test.
This is a bit misleading as there are overall scores for belt tests and 3 individual scores within every belt test. You can get an A overall score with a B (or even C) in an individual test.
The ultimate ninja achievement unlocks if all individual tests have A-,A or A+ scores. So all 30 tests (3 for each belt) must end in some A.
Getting As in everything is hard mostly because you can't redo belt tests after getting the score. In my opinion this is a little too much. Yes, you can abandon the test twice (but not on the last part!) but you have to redo 3 sets of trials once more (which is ok for earlier stages and insane for later). And if you fail at A in the 3rd part of the belt trial you can't do anything.
What you can do is to backup the save file of the game. It is located in the main directory of the game and is called "save.dat". You still have to perform excellently at all 3 individual trials for all belts but if you fail you can always restore yourself back to pre-belt stage and repeat the belt test.
Also you only have to do 5 of the 6 sets of tests before trying any belt test. This also benefits you in a certain way - you won't get the test you skipped for this belt test. E.g. you skipped Katana stage you won't be fighting Onis for the belt test. I always skip Shuriken test because I am clumsy at controls for this stage.

Good advice for the later stages (mostly the last one)
Shuriken
- never did that, seems weird
Hashi
- well, that is f*cking hard in the last belt test (so I just quit when I saw that)
Koi
- the easiest and very easy even for the last belt
Katana
- hard but doable, nevertheless there is a sneaky cheat with pause menu
Hotaru
- seems to me as impossible without pause menu cheat
Nunchaku
- not very hard

Now the pause menu cheat:
You can actually pause the game with space anytime during the belt test. So in the game based just on clicks, which Katana and Hotaru (fireflies) are, you might use this trick. Katana is astonishingly easy - you can kill every last one of them. Hotaru is at least doable - I actually did that twice, with result 27 in both cases, which is the worst possible result for an A (A-).

How to do the cheat? Easy, just pause the game with space. Click on continue and immediately hit space. For Katana it is not necessary to do that immediately, just whenever there is an enemy. For Hotaru it has to be immediately, since you don't know when the next firefly shows up. When in the pause game menu you have all the time in the world and you might see what is going on. For Katana you see if the enemy is attacking and what kind of attack it is, so after clicking on continue you immediately hit the corresponding defense. For Hotaru, whenever you see a firefly hit continue and immediately another left click and then immediately space/pause.

One really good trick:
If your mouse allows that, you can actually bind left+right click on one button. It is super helpful...

Ok, so Strike down your demon achievement is quite, how to say it politely, stupid. I made a script for left-click every milisecond and it still took me several hours to get it. I just left the mouse on the ONI BATTLE button and ran it - so each time the round was over the mouse was still located (since it did not move at all) on the button and started a new round. As mentioned, after several hours of my infinite loop I got it...

Don't have a clue how to get it in some normal way if I am not counting pure luck and really slim chance.

The Ultimate ninja is a different story - it's hard as hell and it seems impossible but with the save file backup it's doable. Without the save file "cheat", I don't think there is someone out there who could do it apart Chuck Norris.

And yes, blocking with both buttons is dumb... To be honest, there are more of the game mechanics which are not good for mouse+keyboard controls. On the other hand, VR could be quite fun!

i accidentally beat the last test and had to start over from white. there was no going back

got the " strike down your demon " ach the way you said it.

Only thing is i was yellow and i did not spam the key i only block once

it's like the 2nd demon strike almost the same time the 1st demon did.
