Introduction
I noticed that there's still no guide on how to obtain all the achievements, so this is my attempt at making one. Disclaimer: I am terrible at this game and there are almost certainly better ways to do everything. Shattered PD is a game of many paths, so if you're struggling with my methods feel free to seek other guides or invent your own. If you have any suggestions, I'd be happy to add them.

I'm going to assume that by the time you're reading this guide you know how to handle ultra-basics like killing snakes and reading your journal, but I'll try to be as generous as possible.

Happy rat punching!
Part One: Unlocking the Heroes

There are five unlockable classes: the Mage, Rogue, Huntress, Duelist, and Cleric. Each has a corresponding badge.

For the mage: You'll find three scrolls of upgrade in every area (5 levels) of the dungeon, so you can easily find and use one within the first few levels.

The rogue: You'll need to perform 10 surprise attacks in one game. This is most easily done by luring enemies into doorways or around walls and then hitting them, and you can do this repeatedly even on the same enemy. You can also throw items at sleeping enemies.

The huntress: You'll need to attack 10 enemies with thrown weapons. You can use the warrior's starting stones or the rogue's daggers plus any items you may pick up early on (fishing spears are pretty common).

The duelist: Equip a tier 2+ weapon with no strength penalty. This means you'll need to find a tier 2+ weapon (you can check by identifying it) and either upgrade it or find and drink 2 strength potions. There are 2 strength potions in every section, so this can also be done before the first boss with a tiny bit of RNG.

The cleric: Find and cleanse the curse on a piece of equipment. Cursed equipment is bountiful in the world of Shattered PD, but for some guaranteed cursed items you can try picking up something dropped from a tombstone or by killing enemies on the sacrificial altar puzzle room. Scrolls of Remove Curse are random, but you'll generally find at least one before the first boss. Alternatively, if you find a really good item early on, you can use your Scrolls of Upgrade to cleanse the curse.
Part Two: Befriending Death

To earn the "Yet Another Sad Death" badge, you'll need to die at least nine times, and there will be no shortage of options for each sub-badge.

Burned: Fire traps are abundant. Try drinking a potion of liquid flame. You could even find the wand of fireblast or a firebloom seed.

Poisoned: You can get poisoned early on by traps or the Gnoll Trickster if you get the ghost quest on the right floor. Cave spinners in the third area might be the most reliable way, since they'll keep poisoning you if you don't die the first time.

Suffocated: Die from toxic gas. Toxic gas spawns in random puzzle rooms, sometimes by traps, cursed armor, or you can find a potion and throw it near yourself.

Starved: Die from starvation. Go somewhere without enemies (pre-boss fight on floor 5 is a good choice) and wait or run around in circles until the inevitable happens.

Pancaked: Die from fall damage. Go down floors until you find one with ledges and jump off.

Zapped: Die from an enemy's magic attack. After the first boss fight in the second area, you can look for the DM-100, wake it up from a distance, and wait to meet the reaper. Dwarf warlocks are another great choice if you've made it that far.

Friendly Fire: I did this one by using an upgraded rod of lightning at close range (unintentionally), but there are no shortage of options. Freely using cursed wands would probably be a fast way to get this one.

A Worthy Sacrifice: Die next to a sacrificial fire. Find a sacrificial altar puzzle room and either lure an enemy in to put you out of your misery or wait.

Deadly Misstep: Die to a grim trap. This is the only one that's even marginally difficult; you'll have to get all the way down to the demon halls (final area). You can examine the grim trap to make sure it's the right one, and then just step on it.

After you've done these, you should get the "Yet Another Sad Death" badge.
Part Three: Side Quests

These are the achievements that you can earn on the side while pushing for the end-game and working your way up.

  • Gourmet
In the introduction I said I'm not good at this game. Cooking is the exception. To get to the max level of Gourmet, you'll need to eat 50 times in one game.

You'll find your default food ration on every floor for a total of twenty. The shop always sells two mini-rations, if you buy all of those you're up to 28. You can find raw meat by killing crabs, cave spinners and piranhas. Roast the raw meat with fire traps or potions. Freeze it the same way. You can use the Horn of Plenty to store food and eat it, but you honestly don't need it. Blandfruit may spawn in hidden rooms, as well as cooked meat. Sometimes you'll randomly find pasties or get them as drops from later level enemies. The Huntress and Rogue also both have abilities that let them find hidden food early on if you're struggling.

Speaking of piranhas...

  • Unconventional Angler

Kill 6 piranhas in one game - you can do this easily with any thrown weapon or the Huntress's bow. If you find the ethereal chains, you can also "fish" them out of the water, or blast them out with the wand of blast wave. You can also try teleporting them out with seeds of fadeleaf, but this is inconsistent and might just teleport them into other water.

  • So Many Colors

Have 10 or more buff/debuff icons active at once.
There are almost certainly several ways to do this, but the easiest for me was:
Play as the Huntress, collect seeds without using them until after beating Tengu (boss #2). Choose the Warden class. Step on a bunch of plants to collect beneficial effects. Add potions if needed.

  • Grim Reaper

Defeat an enemy with a grim weapon. You can rarely find grim weapons by luck, but luck and I are not friends, so I recommend infusing a scroll of upgrade in an alchemy pot to get a Scroll of Enchantment. This will let you choose between three enchantments. It might take a few tries, but you'll get it eventually.

  • Safety Hazard

"Rock gardening teaches the warrior to manipulate his surroundings and use them to his advantage."
~ Piandao

Defeat 10 enemies in one game with the help of terrain hazards. You should do this every run. If an enemy is closer to a trap than you, toss a random item at the trap. Throw hazardous seeds in narrow corridors. Use the ethereal chains, repel enchantments, or the rod of blast wave to push and pull enemies into chasms. You're seriously missing out if you don't use the environment as much as possible; it eliminates hazards for you and preserves your resources.

  • Pack Mule

Fully expand your inventory - at each shop, there's an inventory expansion item (potion bandolier, scroll holder, magical holster). It varies based on what items you have the most of in your inventory, but if you make it all the way to floor 15 you will have the option to buy all of them.

  • Monster Hunter

Defeat X enemies in one game (maxes at 500). In most of my winning runs, I have killed 400+ easily. Take your time on each floor and kill all the enemies you can without running away from them. You could intentionally step on summoning traps to get extra, but I really doubt you'll need it, especially if you choose to ascend with the amulet.

  • Treasure Hunter

Collect X gold in one game (maxes at 15,000). Again, I think by just thoroughly clearing each floor and solving all the puzzle rooms you'll end up getting this one. Pick up all the items you find and sell them to the shop keepers or alchemize them. Kill lots of enemies and look for secret rooms. The thieves' armband can help you "save" money at the shops if you find it.

  • Pacifist

Clear a floor without defeating any enemies. I don't remember getting this one so I'm not sure if you can just skip to the next floor, but the rogue's shadow cloak and skills should be handy for staying out of sight while you sneak around the level.

Part Four: Self Improvement

  • Bodybuilder

Reach X points of strength (maxes at 20)
Since you start at 10, this will require you to find the 2 strength potions in each of the five sections. Take your time searching and this should be no problem.

  • Enchanter
Acquire an item of level X or higher, (maxes at 15)
Similar to the above achievement, you'll need to find (most) of the scrolls of enchantment. There are three per section for a total of 15, so you could either dump all of them into one item or find an item that already has a few upgrades. You can also get some upgrades by using the Troll Blacksmith's services (upgrade an item that's under +2 or merge two items for a free upgrade). I've done this twice that I recall, once with the Rogue and putting all points into the Assassin's Blade, and once with the Warrior and the Runic Blade, but there are many ways you could make this work depending on your preferred play style.

  • Adventurer

Reach level X (max 30)
You'll get this as you reach the end of the dungeon. I don't know if I've ever gotten to floor 25 at less than level 27 or so. If you're having trouble getting to the bottom, there are a huge range of strategy guides online that are far better written than I could ever dream of creating. When I was learning the game, I tried to focus on what was killing me the most and reduce that. If you keep starving, are you taking too long on floors? Did you know the potion of cleansing cures hunger or that you can mix shattered honeypots and healing potions for a potion that restores health and hunger? If you keep triggering traps, are you remembering to search the area and using Aqua Brews to their full potential? If you keep getting killed by snakes, have you read the tutorial?

For the last couple of levels, you can try looking for potions of experience (sometimes lucky random from alchemy, sometimes in shops, sometimes dropped) or grinding extra enemies.

On that note...

  • Alchemist

Craft X items via alchemy in one game (maxes at 35)
Alchemy is a game changer. Don't ignore it! Turn items you don't use into ones you do. Every potion and scroll can be converted into elixirs/exotic versions/etc. You can turn three seeds into a random potion. Scrolls can be turned into runestones. Make bombs if that's your playstyle. You can upgrade potions to make you temporarily immune to fire, frost, or toxic gas. Late game when you've got all the armor you want, turn your scrolls of identify into scrolls of divination to find remaining potions/scrolls/rings. Try throwing things in the pot and see what happens. Use it to clear out your inventory (seed + runestone = alchemize).

  • Dungeoneer

Play 10/25/50/200/1000 games OR win 1/3/5/10/25 games

You'll get one or the other if you keep on tryin'. Once you've won a few games, you'll start learning what it takes to get to the bottom. Or if you don't, you can keep grinding for those 1k attempts.

Part Five: Journaling
There are several achievements that come from completing your journal. I have already written another detailed guide on how to obtain them, so I will only cover them briefly here:

  • Well Equipped
Fill in one catalog entry for every type of equipment in your journal

You'll get this one as you go, just keep picking up items and using them to identify them.

  • Fully Stocked
Identify every regular potion and scroll in one run

Scrolls of divination are your friend here (infuse scrolls of identify in the alchemy pot). Or, if you're feeling really bold, you can try just drinking/using all of them.

  • Big Game Hunter
Fill in ten catalog entries for rare enemies in your journal

Rare enemies spawn throughout the dungeon randomly, but can be increased with the rat skull and mimic tooth trinkets.

  • Not So Intimidating Now
Learn the true name of the King of Dwarves

This one comes from obtaining all the journal entries in the Dwarven Metropolis, which I believe has to be done over multiple playthroughs. It's not particularly difficult though, just look for journal entries as you go along and pick them up.

  • Researcher
Fill in X catalog entries in your journal (maxes with all)

Examine everything you see, use all the items you can, utilize different trinkets for finding rare items, use my other guide if you need more help. Good luck!
Part Six: Double Triple Bossy Deluxe
There are several achievements related to beating the bosses.

  • Slime Janitor and Spotless Victory + (Jack of Many Trades)

Defeat the boss and the end of the sewers / without letting it use its charge attack or heal in water / (defeat it with every class)

Goo is a pretty easy boss once you've met it a few times. Explore a little bit to find some doors or back around the free-floating walls in its chamber. When it starts to charge, back out of range and hit it with ranged attacks if you can. For the challenge, I used the rogue, lured it away from the water before attacking, and went invisible every time it was about to charge.

  • Prison Warden and Gotcha!

Defeat the boss at the end of the prison / without getting hit by any traps or hazards

Tengu is a little more complicated. Use ranged items if you have them to avoid having to walk over his traps (or memorize the pattern if you're really hardcore). Potions can be helpful to do lasting damage as he teleports around the room. All his second phase attacks can be dodged by running away from them with the right timing. Take your time and scope out the situation before moving, turns are indefinite. I did this challenge with the mage when I got a decent wand, moving only to dodge traps.

  • Metal Scrapper and Outmaneuvered + (Jack of All Trades)

Defeat the boss at the end of the caves / without getting attacked or damaged while it is supercharged / (defeat it with all subclasses)

DM-300 is my least favorite boss, but it's not horrible if you know how to beat it. I used the rogue to do the challenge here, going invisible so that I had time to carefully pick my away around the traps without getting beaten to death, but you could also use potions of haste or stones of blink or a dozen other ways. Attack the pylons from a distance or time your movement to avoid their shocks. Try not to lead it over the outlets on the floor.

For the Jack of All Trades achievement, you'll have to come down with all twelve subclasses and you will inevitably dislike some of them. Practice makes perfect. While you're working on that achievement, grab:

  • Gladiator's Fury
Reach a 10-hit combo

With the gladiator subclass, find an area with clustered enemies, kite them through a doorway and beat them until you reach +10.

  • King Conqueror and No Weapons in His Presence

Defeat the boss at the end of the city / without ever directly attacking him with weapons or wands
The Dwarven King is my favorite boss but it's also one of the most challenging. For the challenge, he can't handle your potions, traveler (last terrible reference I promise). I used the huntress so that I could use seeds to have easy immunity to the effects and then blasted him with all the harmful potions I had saved up. The rogue would also be a good choice here, since you can hit him and then become invisible and stall while he takes damage. For the regular fight, learn which enemies spawn based on the particles that appear and prepare. I take out the warlocks first to avoid their range spam, then the monks, and so forth. Once you deal lethal damage to the king, you can become invisible and wait for him to perish.

At some point on your boss quest, you'll probably manage to get:

  • Taste Vengeance!
Defeat a boss while holding the signature item of a hero who died to that boss.

After dying, you'll find the remains of your hero on the subsequent run. Pick up their signature item (remnant of seal/cloak/bow/etc) and don't use it. Beat the boss that killed them to earn this one.

If you manage to beat the final boss and make it all the way back to the top with the amulet and that item, you'll earn:

  • You Made It Too

Bring a fallen hero's signature item to the surface
(More on that in the next section).
Part Seven: Beating the Game
  • Victory!
Obtain the Amulet of Yendor

Many wise words have been written about fighting Yog-Dweza. If you're really stuck, don't limit yourself to my guide. And don't get discouraged if you die a lot, you'll get better over time. Eventually, you will consistently make it to Yog-Dweza and then defeat it. Try to conserve at least a few healing items (but don't get too greedy - you can't use anything if you're dead). Also, when you're passing through the demon halls, stop to take out all of the demon spawners to make the fight easier. It can't directly attack you, but it can attack with beams, summon larva, and summon 3 fists (one at a time).

If you've saved any honeypots, these are an effective and humorous way to handle Yog and the larva. Toss 'em all over. The first part of the fight isn't too tough - just keep stepping out of the beam, hitting Yog, and dealing with the larva as you please. They're more of an annoyance than an actual danger. After you've done a certain amount of damage, Yog will begin summoning fists.

There are the Tank fists (rotting/rusted), which take damage over time; the Environment fists (burning/soiled), which will spread fire or plants; and the Ranged fists (bright/dark), which will darken the environment, hit hard from a distance, and escape you after taking a certain amount of damage.

For the rotting and burning fists: consider making yourself an Elixir of Toxic Essence and/or an Elixir of Dragon's blood. These will give you immunity to the burning and poison effects while you fight. The latter elixir is also useful for the soiled fist, as your burning effect will keep it from surrounding itself with grass.

For the bright and dark fists, hide behind a pillar or wall and make them find you so they can't attack you with ranged. Once they're up close, stay next to them while you pummel their health down.

For the rotting and rusted fists, you'll probably want to use a cat and mouse strategy - hit them, dodge/hide while the damage ticks down, rinse, repeat.

Scrolls of Prismatic image (the exotic version of the scroll of mirror image) can be helpful for giving you a companion to damage the fists. Bombs and thrown weapons of all sorts are useful for attacking the fists you can range. There are so many viable ways to beat Yog-Dweza you could write a guide on that alone, so I'll leave it at this for now. Once you've defeated it, you can proceed to grab the Amulet of Yendor.

After a lot of practice, death, and punched rats, repeat this with each of the six classes to earn:

  • Master of Many Trades

Now, on to ascending:

  • Happy Ending
Take the Amulet of Yendor to the surface

For your first ascent, kill enemies to weaken the curse on the amulet. You'll get a message in game when the curse has been sufficiently weakened. Afterwards, dodge enemies, conserve resources, and find the stairs back up in each level. Scrolls of Passage will no longer teleport you back to the shops, but you can sometimes use Seeds of Fadeleaf and Scrolls of Teleportation to teleport yourself to a location closer to the stairs. A better use would probably be to turn your SoT's into stones of blink and use them to quickly cross floors. You can use any remaining items you have at your disposal; you won't be needing them after you ascend. Scrolls of Terror/Dread can keep enemies away from you after you've weakened the curse, Scrolls of Retribution/Blast can clear out groups of enemies, and so on.

Part Eight: Challenging Yourself

There are five badges left to complete at this point, and they are all exceedingly difficult.

  • Pacifist Ascent

Take the Amulet of Yendor to the surface without ever reducing the severity of its curse

This means no killing enemies on the way up.
First, the amulet will summon nearby enemies right up to you.
Then, enemies will move twice as fast.
Next, your movement speed will be reduced and capped, so no running up really fast with potions/rings of haste and the like :( Luckily Seeds of Swiftthistle still freeze everything if you have some and get stuck in a pinch.
Finally, the amulet will start damaging you every turn and the damage will increase every other floor.

You will get a brief reprieve on boss floors where you can heal and spam the wand of regrowth for healing items collect yourself. You will need to have stockpiled healing potions and food for the journey up. Stones of blink/seeds of fadeleaf are good for escaping a tricky situation. Invisibility and ethereal chains can also help you sneak around. By the time you're nearing the top, you will be taking 10+ damage every single step, so it's critically important that you have a method to move long distances without taking a ton of steps (stone of blink, warrior's heroic leap, rogue's shadow bomb, etc). If you have any spare scrolls of transmutation, you can infuse them to get an item that lets you recycle potions. I used this to turn all of my remaining potions into healing + invisibility potions. You might be able to create a build around soaking up tons of damage, but I haven't tried.

  • Taking the Mick

Strike the killing blow on the final boss with a +20 or higher pickaxe

I did this one with the huntress, using her bow for my main damage and the pickaxe as backup. To get a +20 item, you will need to save all your scrolls of upgrade, get the pickaxe from the Troll Blacksmith, accept his two free upgrades, use all 15 scrolls of upgrade (+17), then craft a curse infusion (Scroll of Remove Curse + Metal Shards from the DM-300) to get the final +3. You can re-use the curse infusions to get a "better" curse if desired, but you won't get more than +20 in level. I imagine this would also be easier with the melee based characters (Warrior + Duelist).

  • Doom Slayer
Defeat the final boss with all demon spawners alive and the badder bosses challenge enabled

Just like it says, you'll need to avoid taking out the demon spawners and turn on Badder Bosses.
You can't enable challenges until you've beaten the game at least once (why would you?!)
In this challenge, all the bosses will have more health and generally be deadlier. Goo heals faster and pumps up in one turn instead of two, Tengu's traps and abilities are deadlier, DM-300 activates an extra pylon and is stronger/faster, Dwarf King's minions are stronger and more numerous, and Yog-Dzewa will summon two fists at a time and do more beam damage.

I think this challenge sounds harder than it actually is. If you've learned the dungeon mechanics and can reliably beat the boss fights, the same strategies will carry you through, you just need to be more cautious and practice good habits (like not charging in and trying to tank everything). Use ranged damage when you can (Huntress is a favorite of many veteran players for a reason), avoid traps and attacks carefully, and learn from your previous deaths. Yog-Dzewa will still definitely give you a run for your money, but it's very do-able with practice and a few attempts.

  • Champion

Beat the game with 1/3/6 challenges enabled

If you've done the previous achievement, you'll already have the lowest tier of this badge. Sadly, 3 challenges is as far as I can take you, as I'm still working for 6 myself. UPDATE: I finally managed a six challenge run with the rogue! In my opinion, the three easiest are: Hostile Champions, Badder Bosses, and Into Darkness. I play with the Rogue frequently, so darkness makes little difference. As previously described, once you're good at killing the bosses, Badder Bosses will only be a minor upgrade in difficulty. Hostile Champions gives you a 1/10 chance to encounter "champion" enemies, which are buffed up. They can either be killed or avoided. Don't get me wrong, the combination of these three challenges definitely still makes for a tough playthrough, but nothing that you can't handle once you've mastered the basics of the game. I practiced with each challenge individually before stacking them together. Most of the other challenges involve beating the game without/with less of something (healing potions, scrolls of upgrade, armor, grass, food). Your playstyle will impact which of these you find most difficult.

  • Score Chaser

End a run with a score of 5k/25k/100k/250k/1M or higher

Your score is calculated by adding the following factors:
Progression: Did you make it to the bottom? Is your hero max level (30)?
Treasure: How much gold you collected/how much your remaining items are worth
Exploration: Did you fully explore every area/solve all the puzzles?
Bosses: How many bosses did you defeat? Did you get hit by avoidable attacks?
Quests: Did you complete all the side quests? Did you get hit by avoidable attacks in them?

These are all added together and then multiplied by:
Win: Whether you died (x1), died with amulet (x1,5), ended with amulet (2.0x), ascended (x2.5)
Number of challenges x 1.25 rounded up to nearest .05 (1 = 1.25, 2 = 1.55, 3 = 1.95 etc)

To reach 1M, you will need to ascend with the amulet and a max level character, have cleared as many areas as possible/fully explored, and have several challenges activated.

Good luck!

Great guide
