This guide contains the locations for all notes and mostly spoiler free help on how to obtain endings for all 3 chapters.

Introduction + Overview
You play as a priest called John Ward, returning to a house in the forest after a year to do something 'not approved by the Vatican'. There are three chapters to this game, however Chapter 2 has a prologue which is only unlocked after beating Chapter 2. Some things to remember are:
  • Unless stated elsewhere, John dies after the first hit
  • Your only defence is a crucifix (and annoyingly you can't move whilst using it)
  • You gain notes by exorcising objects in your environment. These notes either tell the story or give background knowledge to events that have happened or are going to happen soon
  • You cannot manually save (there are three exceptions to this rule), however, auto save is used for key events like ending determining choices or during death
  • Most chapters have 'secret' boss fights or endings, which i will list in the guide to the best of my abilities
I have tried my hardest to keep this guide as spoiler free as i feel it can be without being confusing. However, if you feel this guide is too confusing or reveals too much too early, please feel free to let me know so i can either reword certain segments or add a spoiler tag.

Chapter 1
For chapter 1, there are 20 notes you can collect and five endings that give you specific notes (these 5 notes are not counted in the note total for the chapter and cannot be looked back upon in the note collection). 9 of these notes are found in the forest, with the remaining being inside the house.I will also be listing the notes in the order we come across them, so they may be slightly out of order

The forest
Below will be an image of the external map as the player sees it, so the descriptions will be quite vague (credits to the Faith Wiki for the photo)

  • Note 1 is found above John's car when you spawn in (please don't venture into the road unless you want to be greeted with one of the most humbling mortis you will ever have)
Once leaving this area, you will be randomly targeted by the 'Chupacabra' whilst moving between areas. Each time you repel him, he gets slightly faster, however can target Deer if there is one closer to him than John is when he spawns. On your 10th time repelling him, he drops Note 9 (if you collect this after an auto-save [like inside the shed] and die before reaching the next auto save, you will still be targeted until he drops the note again)
  • Note 3 is found by exorcising the dead tree closest to Johns car
  • Note 2 is found by exorcising the well
  • Note 7 is found by exorcising the stone ruins far west of the well
Continue to the door of the house
  • Note 6 is found by exorcising the graves next to the house
Head towards the shed
  • Note 4 is found by exorcising the water puddle near the shed
  • Note 5 is found by exorcising the circle of bones near the shed
  • Note 8 is found outside of the shed
Go inside the shed, grab the item and make your way back to the house
Inside the house
  • Note 10 is found when you enter the house
  • Note 11 is found by exorcising the family photo in the same room
  • Note 12 is found by exorcising the drawing on the fridge in the kitchen
  • Note 13 is found by exorcising the rubber duck in the bathroom
  • Note 14 is found by Amy's desk
  • Note 15 is found by exorcising the doll in Amy's room
Go back downstairs to the kitchen and descend the stairs. Now that you're in the basement, make your way around to the lower left corner, picking up note 16 on the way
  • Note 16 is found by exorcising the crib in the basement
  • Note 17 is found in the top left corner of the lower left area
After an event in the basement, make your way up through the bathroom, down the stairs, through the downstairs bedroom, the living area then to the kitchen. Be aware that through this event, you are being trailed and can see when she will strike by the symbols on the floor. exorcise in that direction and then continue (four Amy's may appear, you need to exorcise the real one, which can be told by her flickering at a different rate to the fakes when first surrounding John).
Once the prompt appears, head up to the attic
  • Note 18 is found in the attic walkway
Once in the attic, a boss fight will commence in three stages. For the first stage, immediately aim your crucifix at her to dwindle her health before she attacks. Stage two is essentially a repeat of downstairs, apart from the fact that she charges you from the opposite direction of the real vessel. For stage 3, stay away from her path as you cant attack her whilst she moves. Once she stops, make sure to stay a distance as she will charge you if you're in her circle.
After the fight, make your way back downstairs
  • Note 19 can be found on the second floor hall only if you have collected notes 1-18
Go downstairs and you'll find a gun. Head west into the Parents' bedroom and shoot the mirror three times (the gun disappears after one shot, but respawns if it is used for this purpose or shot accidentally).
  • Note 20 can be found after defeating the boss

Endings
Chapter 1's endings are determined by your actions from this point onwards and what you do with the gun.
  • Ending 1: Murderer - Grab the gun and head south to the first 'path' to west, keep going until you find her and shoot. Return to your car
  • Ending 2: Father and son - Grab the gun and go to the shed. Shoot the grey figure resting there. Return to your car
  • Ending 3: The Offering - Grab the gun and go to the ring of stones south west of the well and shoot the animal. Return to your car
  • Ending 4: The Hunter - Grab the gun and go east of the fountain to shoot the Deer. Return to your car
  • Ending 5: When Faith Endures - The 'True Ending' for this chapter. Grab the gun and return to your car without shooting. Shoot the Chupacabra when prompted and get into your car.
Chapter 2
Endings
So for this Chapter, there are three endings, with ending three (and Notes 9, 16 and 23) requiring three extra steps. I will add these steps when relevant using a spoiler tag so it is easy to play without, but easy to know where to fit the steps in. There are 27 notes in Chapter 2, so its better to pick them up as you go, meaning the order collected may be slightly off.

  • Ending 1: Go Forth With Faith- The 'true ending'. Complete the chapter normally without meeting any of the extra steps (quite easy as the extra steps are out of the way)
  • Ending 2: Road To Redemption- Complete the chapter having only done one or two extra steps
  • Ending 3: Initiation- Complete the chapter with all three extra steps made

Garcia's crib
  • Note 1 is found next to Father Garcia's bed
Head down to the basement, looking at the photos around the room as you go. After the cutscene, walk towards the staircase, then head outside the front door.

The Graveyard
Follow the 'natural' path to the gates
  • Note 27 is found north of where John spawns
  • Note 2 is found by exorcising the tastefully decorated tree
  • Note 3 is found by exorcising the misaligned headstone just through the gate
  • Note 4 is found just outside the SAVE tomb
As you probably could have guessed, this tomb allows you to save by entering it. This will become helpful later. Approach the mirror at the end. After the scene, exit and go to the west.
Make sure you look at the statues and the direction they are pointing, as its the solution for puzzle to the north. If you are confused, the solution is: Right, up, left, up.
  • Note 5 is found by defeating the demon
Once you have defeated this enemy, go down two areas for your next challenger
  • Note 6 is found by defeating this demon
Once you have found both demons on the left side, walk past the SAVE tomb to the right side, where you will see a closed gate and a purple skull on the other side of the wall. Walk down and around to the skull.
  • Note 7 is found by exorcising the misaligned tomb on the way to the skull
Exorcise the skull and enter the gate
  • Note 8 is found after defeating the demon
Return to the save tomb and watch the cutscene
First condition for ending 3: Now that John's bleeding, travel back to the circle of stones where you first spawned and draw a pentagram in the blood starting from the top left and going down. Ward off the demon for note 9. After this, follow the next steps as usual
Make your way north from the SAVE tomb to get to the exit gate.
Snake Meadow Hill Church
  • Note 10 is found in front of the church
  • Note 11 is found by exorcising the scarecrow in the corn area behind the church
  • Note 12 is found directly when you enter the church
In the church, there is a puzzle that needs solving relating to the candles and an entity that can kill you. Basically, the candles represent the layout of the church areas. There should be two candles lit, one where John is and one where the spindly lady is. Venture the church finding notes until there is only one candle is lit, meaning shes in the same room you're in. You need to locate and exorcise her before she gets you.
After this, the church basement will be open for business, however there are quite a few notes in the church and after the basement opens, you are no longer being hunted.
  • Note 13 is found by exorcising the purple skull next to the confessional booth
  • Note 14 is found by entering the confessional and exorcising the virgin Mary painting
  • Note 15 is found by exorcising the third stained glass window by the altar
Before entering the basement, we have the opportunity to do the second condition for ending 3:
You should have already been in the confessional by now, you go outside to the cornfield and a child should approach you. Don't look in its direction or else it will run away. Lead it to the booth. Note 16 is obtained after defeating it.
Go down to the basement
The Basement
  • Note 17 is obtained when entering the basement
This puzzle is quite obscure (at least for my limited brain capacity), so im not going to bother spoilering the answer. The note in the room above said that his watch stopped at 2, so imagine the wheel as a clock and start from the symbol that would be 2 on a clock. In the next room there are tiles you need to step on in order starting from '2', which is the 'H' looking symbol onwards.
After escaping the basement, go south and follow the path
  • Note 18 is found in the circle
After a cutscene, go down,right and up to exit the room.
If you're going for ending 3:
Before passing under the bridge, go down until you find a gap in the left wall and travel towards the couple as a spider. Kill the couple (or only one, it doesnt matter). Watch out on the road unless you want to be greeted with yet another embarrassing mortis. Return to the original path and pass under the bridge. If you accidentally pass under the bridge before completing this condition, you can exit to the title screen and press continue on chapter 2, as it auto saves you in the room
Pass under the bridge and walk towards the sewers
Candy Tunnel
  • Note 19 is found by going east once you enter the tunnel and exorcising the skeleton
  • Note 20 is found by going west from this area and exorcising that skeleton (yes, the layout changes)
Go east twice then down where you'll see 4 pillars with writing. Read it and go east and walk through the room. Don't move. Follow it and climb into the open grate.
  • Note 22 is found by going all the way east within the pipe
  • Note 21 is found by going all the way west within the pipe, it should be behind a walled part
Remember, if you die, you need to find the notes again (not putting this note at this point from any personal frustration or anything..)
After getting note 21, walk east until you find the grate with the green arrows pointing up, go north at this point and follow the path.
Once out of the tunnel, go south and follow the blood.
Collect the flashlight in the corner of the room.
For this fight, you need to locate him in the room and exorcise him as long as you can before he teleports away. Be careful, as he can spawn very close to you. When you first grab the flashlight, you can do some easy damage by attacking him whilst he's stationary at the entrance.
Once he's defeated, go north into the pipe
Sanctum
You will come out into a dark room, go right and locate a painting behind the altar and exorcise it. Be aware there are enemies in the room too, so use your flashlight to ward them off and be swift. This will open a door to the right where you can find the key for the main door.
  • Note 23 can be found in the room to the north only if you have completed the 3 conditions for ending 3
Go back to the starting room and use the key to unlock the door
Protect Garcia (or don't, he only needs to be alive for the good ending)
She has three stages, each one a pain in the ass. For the first one, aim your cross at her, making sure to ward away any thralls that try to attack Garcia. For the second stage, use your flashlight to locate her then ward her off. For the third stage, just keep Garcia alive by warding her whenever she flashes to avoid her charging at him.
Home
  • Note 24 can be found by John's bed
  • Note 25 can be found by the front door (excluding ending 3)
  • Note 26 can be found in the kitchen
Chapter 2 (Prologue)
This 'chapter' has a score system, which isn't present in the others. The way to get the highest score is to get the second ending.

Cornfield
When you spawn in, go east and investigate the car. Go to the east screen then come back (this boosts your score for the chapter). Follow the trail left behind. Venture the fields until you see the stalks being crushed in your direction and point your crucifix in it's direction. Ward it away three times then proceed north of the scarecrow (warding it wont do anything)
  • Note 1 is found by exorcising the dog
  • Note 2 is found by exorcising the statue under the gazebo
Proceed up from the gravesite and enter the church
  • Note 3 is on the floor when you enter the church
Go west into the next room and exorcise the chalice.
  • Note 4 is found once the demon is defeated
  • Note 5 is found on the cover of the basement stairs
Go east from the stairs and exorcise the painting of the Virgin Mary
  • Note 6 is found on the cover of the basement stairs
Go west from the stairs and exorcise the purple skull (if it isnt purple yet, go exorcise Mary again)
  • Note 7 is found on the cover of the basement stairs
Go north and exorcise the third stained glass window. Retreat quickly to the basement in order to not get attacked. Grab the flashlight and go south.
  • Note 8 is found once the demon is defeated
Exit the basement. Here you can get one of two endings.
Ending 1: HERE I AM- Once out of the basement, head up towards the stained glass windows
Ending 2: GET OUT- Necessary for getting 666 score. Go outside to find the scarecrow and exorcise it until its eyes aren't red. Then run down to Johns car, set yourself on fire, then run into the scarecrow.
[Chapter 2 will now start]
Chapter 3
Chapter 3 is quite a long chapter with a lot of easy to miss notes and fights. There are 44 notes to obtain in the chapter, with a few only being obtained in ending 2 and ending 3. Again, i will be listing these in the easiest order to obtain them without going too much out of your way, so it may be hugely out of order (mainly the second day). Seeing as these endings are determined throughout the chapter, i will vaguely list how to get them now
Ending 1: A New Purpose- Play through the chapter without doing the extra steps
Ending 2: A New Journey- Contains notes 8, 27, 28, 41 and 42. Play through the chapter with the extra steps
Ending 3: A New Vessel- Contains notes 43 and 44. When going to a location to investigate, immediately get back into your car and drive back home every time
Before you start, there is a scene with a needle within the chapter, so if you don't like that sort of thing, make sure you untick the setting before starting the chapter
I'm also going to explain how to proceed with ending 3 here as it doesn't require you to actually play the chapter and you get notes from it: on the say of the profane sabbath, go to the basement to pick up the key to the forbidden room and unlock it to get note 43. Once teleported, go to the basement to get note 44. Go to the twins room for the ending
Day 1: Clinic
Follow Father Allred into the basement, then lead the way to the bottom left area.
  • Note 1 is found in John's inventory from the start
  • Note 2 is found next to John's bed
  • Note 3 is found in John's kitchen
  • Note 4 is found by John's front door
Once you get Garcia's note, go outside and get into your car to drive there (you can also creep on your neighbours by going east or west, what you see will change based on the day)
Once you arrive, go to the area to the west and go through the gap in the fence
  • Note 5 is found under the poster
Go right then sneak through the broken window
  • Note 6 is found in the Clinic Lobby
  • Note 7 is found by exorcising the IV drip in the hallway south of the lobby
Once in the room with the ultrasounds, go to the west to collect the crowbar then return to the boarded up door
When you regain control, move behind the curtain closest to John to hide, then leave through the door once the demon is focused on the bed across from him. When down the corridor, hide behind the cluster of beds near the closed door. Once the demon leaves, go towards the open door.
Make your way to the lobby and prepare yourself, this guys ai is ass, so its best just to run around the room avoiding them.
For those going for Ending 2: Go back into the clinic after being led outside by the police officer, head back to your gurney and get back on it. Move to the closed door and wait. Enter the door. For this fight, the babies only slow John down as he exorcises the mother. The only danger is the babies that crawl towards the corners after departing from the mother. Note 8 can be collected in the hallway after the fight.
Once outside, go home.
Grab the bibles where they spawn, then escort the parents upstairs when asked. Go back down to your partner.

Day 2: Apartments
  • Notes 9, 10 and 11 will be found at John's front door
Get into your car and travel to the apartment building
  • Note 12 can be found in the west part of the first hallway by the mailboxes
Go to Tiffany's apartment (3B) and grab the key to Lisa's apartment
Go to Lisa's apartment (5A)
  • Note 14 is found by the elevator on the 5th floor
  • Note 13 is found in Lisa's bathroom
Grab the blue key in the bathroom and exorcise the seal on Lisa's room before leaving.
  • Note 17 is found by the elevator on the 6th floor (Note 18 is the second part to this note and is necessary for ending 2, however i'm leaving that till after we finish investigating Lisa's apartment, as it requires summoning an enemy that randomly attacks in an otherwise mostly peaceful segment)
  • Note 25 is found be exorcising the clock next to the west stairs on the 6th floor
  • Note 15 can be found in apartment 8A
  • Note 19 can be found outside of 8B
  • Note 26 can be found by exorcising the pumpkin outside of 9B
Go to the 10th floor and go west to the door with an M.
  • Note 20 can be found in the maintenance room
Place your crucifix into the dumbwaiter then take the elevator to the first floor.
  • Note 21 can be found on the 4th floor by the elevator
Continue east and pick up the camera. This will be your light when traversing the dark. Continue down the stairs and enter apartment 3B. Go into the bedroom and enter the tunnel at the side. Once you exit apartment 3A, go down the stairs and enter 2A. Go down the staircase under the table. Navigate the maze and exit 2B. Go down the stairs and it should be a straight path to the basement. Once in the basement, go to the area east to collect your crucifix. After collecting it, go east and exorcise the mask on the floor.
  • Note 22 can be found in the corner of the basement
Don't exit the next room without exorcising the demon, as it will kill you. Exit the basement.
Go to 2A and exorcise the blue blade
  • Note 16 can be found in 2A
  • Note 24 can be found in 2B by exorcising the eye on the wall
  • Note 23 can be found by exorcising the altar in the tunnel between 3A and 3B
Go to Lisa's bedroom in 5A.
For this fight, you need to exorcise Lisa and find out which vessel the demon possessed before running out of time. Running out of time causes John to be possessed which can cause him to kill Lisa if he touches her. The game continues if Lisa dies, however i think she is needed for ending 2 and you don't get the achievement for investigating the apartments if she does. If she dies and you want her alive, DON'T EXIT THE ROOM or else it'll save the game with her dead.
Follow Lisa out of the bedroom and talk to her.
  • Note 18 can be found by summoning the elevator friend and exorcising him 8 times. You summon this demon by repeatedly pressing the button for the 7th floor in the elevator until he pops up.
For those going for ending 2: Now that you have notes 17 and 18, read the numerals at the bottom in order to get a list of numbers: 1, 4, 2, 6, 2,10, 5, 1. These are the floors you are going to push in the elevator in order to get to the secret boss of the apartments. Don't talk or look at the lady and if you did it right, instead of going to floor 1 at the end, you should be at 10. Once you are on the secret floor, go into the apartment on the right. Now that you have defeated the boss, note 28 can be found once you leave the room and jump down the elevator. The building should be back to normal, by that i mean actually have a 7th floor. Note 27 can be found by exorcising the body in 7A..
Exit the building.
Day 3: The Nursery
For this segment, go into the attic (although there's a cool scene if you leave the house and try to go to the car)
  • Note 29 can be found by John's front door
Get in John's car and drive. Once there, go west and up the alleyway around to a gap in the fence
  • Note 30 can be found in the playground
Go through the door above
  • Note 31 can be found in the room to the south next to the coloured objects
This puzzle requires you to look at all the drawings to find out what colours the star, chalice, moon and bird are in order to proceed. Red Star, Pink Chalice, Orange Moon and Dark Blue Bird. Exorcise the items until they are the correct colour then descend the staircase.
Garyland.
Go through the gates and continue the path.
  • Note 32 is found by exorcising the remains in the next room
  • Note 33 is found at the bottom of the stairs
Stare at the painting for about 30 seconds and walk through the tunnel
  • Note 34 is found in front of the SAVE statue
  • Note 35 is found by exorcising the SAVE statue
This statue works the same as the SAVE tomb in the previous chapter, allowing you to save between fights and gathering items. Go west and read the writing in each corner, then head north exorcise the heads (i have no idea how to solve this, this is just one of the puzzles you fart about with until something happens).
Go downstairs and search around before going to the basement to get Allred (or leave, which gives an interesting scene)
Go east and follow the path past the statue to the next SAVE statue
  • Note 37 is found by exorcising the second SAVE statue
  • Note 36 is found by going all the way east
Play a lovely game of red light, green light
Return to the save statue (be warned that walking past the painting three times causes an enemy to spawn. Killing it is necessary for ending 2, however i kill it on any run cause its really annoying. Do as much damage as you can then change areas, wait for it to approach then repeat until it dies).
Go east again but this time go north until you find a mirror room. Make sure not to let the demon touch your reflection and kill it. This will give you the second item. Return to SAVE the head west to grab the last item. Once you've grabbed it keep trying to leave. Once the thrall spawns, i usually avoid it by wall hugging very tightly.
Place the items into the big statue
  • Note 39 is found at the bottom of the stairs
  • Note 38 is found in the hallway to the north
Follow the hallway all the way down then back up. Follow the red trail.
  • Note 40 can be fund by going north and following the path northwest to find the cabin. It is inside
For this section, there are three creatures to be exorcised to remove the seal and they can be found using the red paths to the east, west and north (excluding the northwest path that takes us to the cabin).
Enter through the door and head north. Choose any questions.
This fight doesn't immediately kill you, as it just knocks him down until he grabs his cross, and is quite simple in structure. Once the fight ends is where the endings branch

Ending 1: Try to follow where Gary went, but due to not defeating any of the unholy trinity, you cannot access the crucible, so you exorcise the stone above it to seal it instead. This ending has 2 different ways it can go depending on whether you killed Lisa or not.
Ending 2: Follow where Gary went due to having defeated the unholy trinity and destroyed to seal preventing you in ending 1. Note 41 is found on the right side of the room you spawn in and 42 is found almost every room you look. Go to the attic.
