You only have to beat Level 18 in order to 100% this game, and your ratings (stars or mortality points) do not matter. All achievements are in the Campaign levels, and you can return to any level after you beat it. If you need to throw large objects, you can use them to break the windows and exit there instead of the using door.
This game took me around 1.5 hours to complete, and it took my friend close to 3 hours.

On Level 2, pick up the pillow and use it to knock out the guy on the left. You need to hit him so that he falls forward into the road and gets run over by the car before he stands up.

Also on Level 2, there are two birds that fly in circles in the air. One of the birds will fly over the balcony. Wait for it, then throw an object directly up in the air to knock down the bird.

Level 5 is the easiest place to get this achievement. Use a large object (the table or the fridge) to knock out the two guys standing in front of the building.

This one is the same as double knockout, just wait until the third guy walks next to the other two. Use the table or the fridge (again) for a bigger area of damage.

On Level 6, you need to hit one of the two ducks in the pond before you hit the target.

On Level 11, stand on the right corner of the balcony and throw something at the roof across the street. Time your throw so that the snow sliding off the roof falls on the car.

On Level 16, there is a crane holding a beam (balk?) on the left side of the bridge. Throw something at it to knock it down. The beam does NOT need to land on the car, just wait until the car runs into it on the road for the achievement.

On Level 19, the helicopter will be on your left when you walk out the double doors. You can throw any object directly at the helicopter to break it, or throw a breakable object (like the washing machine) and hope that one of the pieces makes contact.
