Forgotten Continent Native City+State Names

My attempt to change city/state names to make more sense with the Forgotten Continent mod by Prisoner.

Should Prisoner (or anyone else working on a similar/successor mod) ever wish to incorporate this directly into their mod, they can feel free to do so any time - no need to ask or wait for permission, just let me know.

Should continue to be compatible with future game updates for as long as the game doesn't add any more states, assuming Forgotten Continent is also updated.

I'm not a Native American, nor an expert in any Native American histories, so let me know if you either see an error, or just have a better idea for a name. Just keep in mind that, since the state borders in the game are based on those drawn by European colonizers who didn't exactly respect pre-existing ethnic boundaries or native place names, it's impossible to be 100% accurate here.
