NOTE: If you skip an earlier weapon upgrade and grab the next upgrade for that weapon type, you will actually unlock the previous upgrade instead of what is listed. For example, if you miss the LKW's "Full Automatic" upgrade in Chapter 6, but grab that "Reflection" upgrade in Chapter 7, the "Full Automatic" upgrade will unlock instead. As of now, this appears to be an uncertain fact and requires confirmation.

Lastly, Weapon Upgrades can be obtained at any time and are not restricted to either of the game's timelines. With that in mind, you can always replay a Chapter via Chapter Select, grab the upgrade you need, and quit out to continue another mission. The game saves your progress as soon as you pick up a weapon upgrade.

To obtain Armor upgrades throughout the game:
Spare Wyatt by letting Deathshead take Fergus.
Each Armor Upgrade will increase the amount that armor pickups give you by 10%. It does NOT increase your maximum armor.

To obtain Health upgrades throughout the game:
Spare Fergus by letting Deathshead take Wyatt.
Each Health upgrade will increase your max health by 10%. It does NOT increase the amount health pickups give you.

[Weapon Upgrade - Handgun 1946: Silencer]
This weapon will be replaced in the next mission, so the upgrade isn't super necessary. Regardless, when you get the dynamite, instead of placing it on the large metal doors, take the right path and place it on the large missiles in the small room (alternatively, just use a grenade to blow the missiles up). Jump through the hole it makes in the floor, and follow the tunnel. Partway through, you should find the upgrade.

Weapon Upgrade - Handgun 1960: Silencer
Look for the barricade in the hallway opposite of your starting position in the first area. Above that barricade you can gain access to the roof and get the silencer for the pistol.

Armor Upgrade #1 [Wyatt Timeline]
In the upper floor of the Asylum, keep on going straight from the long corridor and duck to get through the fallen debris. As soon as you recover, head inside the bathroom on your left to find the Armor Upgrade inside a cabinet.

Health Upgrade #1 [Fergus Timeline]
On the second floor of the Asylum, you will come across a small office (near the reception) with a safe inside it. You need to trip the wires to open up this safe and collect the Health Upgrade.

Armor Upgrade #2 [Wyatt Timeline]
While interrogating the ♥♥♥♥ Commander, open up the wooden cabinet nearby and collect the Armor Upgrade.

Armor Upgrade #3 [Wyatt Timeline]
Once you clear the East Section ride on the side of the car for a few moments, head inside the building the now-dead ♥♥♥♥ was standing by. Instead of going to the left room (with the elevator) turn right, going through the door you can lockpick. You will come across a stairway with a crawling space inside the Construction Area. Crawl through this area and get the Armor Upgrade.

Health Upgrade #2 [Fergus Timeline]
While interrogating the ♥♥♥♥ Commander, open up the wooden cabinet nearby and collect the Health Upgrade.

Health Upgrade #3 [Fergus Timeline]
Once you clear the East Section ride on the side of the car for a few moments, head inside the building the now-dead ♥♥♥♥ was standing by. Take the elevator upwards in the left room. After riding the elevator, head up and keep on going straight until you spot a reel of wire on your right before the catwalk. Behind this reel of wire will be another Health Upgrade.

[Weapon Upgrade - Laser Cutter]
Once you've infiltrated the prison, after sneaking through the area with the security cameras, you can find this in the coal loader room (lots of red beams, catwalks, and coal loaders below). On the catwalk, take the small steps to the left down to a lower catwalk. The upgrade looks like a recharge station.

It will later be replaced by the Laserkraftwerk.

Armor Upgrade #4 [Wyatt Timeline]
In the Kreisau Circle, you will see a small room (near the meeting table). Head inside this room and use the computer. When you're done, you'll see this Armor Upgrade on the desk.

Health Upgrade #4 [Fergus Timeline]
In the Kreisau Circle, you will come across a small room (near Max’s bedroom) with a bed inside. Get some rest there and you will find a Health Upgrade on the side-table after waking up.

Weapon Upgrade: Laserkraftwerk - Full Automatic
Once you make it to the hanger you can get this upgrade. You will see a ♥♥♥♥ soldier sitting inside a small computer-room. If you pull open the map, it is located inside the bottom room next to one of the commanders.

Armor Upgrade #5 [Wyatt Timeline]
This Armor Upgrade will be located inside the Main Lobby area. Go all the way to the back side of Café and you will come across a metallic door that leads further into the level. Before going through it, look to the left side of this metallic door to spot a locker containing the Armor Upgrade. You'll have to pick the lock to open it.

Health Upgrade #5 [Fergus Timeline]
This Health Upgrade will be located inside the Main Lobby area. Near the entrace you came through, you will come across a safe in the left side side which you need to open by hotwiring.

Weapon Upgrade - Laserkraftwerk: Reflection
Once you are retrieving the power saw, you will fall down into the sewers. Follow the path until you pass through a hole in a chained fence and pass a smaller room with loot. Instead of heading left for the objective, take a right down the sewer and the upgrade will be near the metal gate.

Armor Upgrade #6 [Wyatt Timeline]
This is the same as the upgrade from Chapter 5. In the Kreisau Circle, you will see a small room (near the meeting table). Head inside this room and use the computer. When you're done, you'll see this Armor Upgrade on the desk.

Health Upgrade #6 [Fergus Timeline]
This is the same as the upgrade from Chapter 5. In the Kreisau Circle, you will come across a small room (near Max’s bedroom) with a bed inside. Get some rest there and you will find a Health Upgrade on the side-table after waking up.

Weapon Upgrade - Laserkraftwerk: Strobe
After you collect the welder, you you will have to knock a ladder down with your laser. Once you go up the ladder look to the right for this upgrade box.

You will find a guitar inside the HQ which you need to play in order to get this Armor Upgrade.

Health Upgrade #7 [Fergus Timeline]
You will come across a bed inside the HQ. Give those fists some rest and you will find the Health Upgrade after waking up.

Weapon Upgrade - Laserkraftwerk: Tesla
After you take the elevator up towards the train, you can find this upgrade in between two crates. The upgrade is a large yellow box and is hard to miss.

Armor Upgrade #8 [Wyatt Timeline]
While trying to open up the passage by using mines, you will see another mine in the same location which will open up a tunnel. Head inside and enter the door at the far end to get this Armor Upgrade.

Health Upgrade #8 [Fergus Timeline]
Once you have raised the water level, swim deep to find a safe suspended in the water. You need to hotwire this safe to get this Health Upgrade.

Weapon Upgrade - Shotgun: Shrapnel Rounds
You will automatically get this upgrade during the chapter.

Weapon Upgrade - Laserkraftwerk: Portable Reactor
Towards the end of Chapter 11, you will unlock a story-based achievement and find yourself in a treasure room. You will need to solve a small puzzle in order to unlock this upgrade.
You can follow the following video for assistance in solving the puzzle.

Weapon Upgrade - Assault Rifle: Rocket Launcher
At the start of the chapter, you will be dropped near a broken train cart. Climb the ladder and enter the train car ahead of you. On the first floor, you will find the upgrade with ease.

Weapon Upgrade - Laserkraftwerk: Targeting Scope
After you clear out the double decker train car of ♥♥♥♥♥, you will climb up to the main train cart. Traverse the beam across to the train cart behind the double decker and cut a hole in the fence with the LKW to get this next upgrade. The upgrade is a large yellow box and is hard to miss.

Armor Upgrade #9 [Wyatt Timeline]
After taking out the Super Soldiers, you need to enter the yellow door nearby to get this Armor Upgrade.

Health Upgrade #9 [Fergus Timeline]
You will see a small room located on your right side after getting down from the helicopter. You need to head inside this small room and open up a safe to get the upgrade.

Weapon Upgrade - Marksman Rifle: Laser Fire
You'll upgrade automatically by killing a solider and grabbing their gun.

Armor Upgrade #10
You will find one of the conveyors obstructed by a metal crate. You need to open up this path and continue ahead to find this Armor Upgrade.

Health Upgrade #10
You need to ascend the stairs located inside the Base's war room which will take you to a locked door. Head left from this room and the upgrade will be inside a safe.

Weapon Upgrade - Laserkraftwerk: Ammo Capacity [Fergus Timeline]
This upgrade only appears in the Fergus Timeline. (As far as I'm aware) this is the only exclusive weapon upgrade.
After you destroy the helicopter, go down to the next floor and you will find the upgrade in the corner of the room.
