Dwarves can get hammered drunk and wander off ledges.. I had a waterfall tavern that killed anyone who fell in.

In one fortress I lost 0 dwarves to the goblins directly but at least 3 because they were pushed off a bridge just like that one during sieges :[

Also Dwarves might try to just run in a direct line away from something that terrifies them, like a kea, try to climb the wall right there, and then fall.
