Level 1
#1/12 Skeleton = After you clear all the roots, there is a small house with a ladder at the side. Climb up, its on the other side of the roof
#2/12 Wraith = Behind a locked gate to the right, just after #2. Not sure where the lever is? But I climbed over the rocks near the cemetery, ran along the fence line and jumped over to get it.
#3/12 Blood Skeleton = After you pass the cemetery, at the smaller mausoleum, turn right behind the fence, its along the rock wall

Level 2
#4/12 Skelemental = After you open the first gate on the left, is it on top of the light fixture. Jump on the staircase and double-dash over to it
#5/12 Knightmare = When you come overlook the massive bridge, and are required to jump down into lava below. It is floating high up in between the pillars, approximately under the centre of the bridge. You can dash across on the pillars to reach it.
#6/12 Gloom Wraith = When you the bridge with the massive axe blades swinging, take a step and there is a smaller, separated lava section. It is floating underneath the bridge. Jump down on top of one of the columns and dash over to it.

Level 3
#7/12 Werewolf = At the castle ruins to the left, jump up on top of them and dash across the rock wall (Where the smoke blocking your path was), climb up the rocks and there a section of the wall that is broken.

Level 5
12/12 Skeleton Crusader = Inside the Castle grounds, where you take the elevator up to collect the gem. Jump/Dash to the building to right of you, drop down one level and there will be a little statue and a lever on the roof. Activate the lever and then drop down to the floor, collecting it underneath.

Level 2
Near where the smoke-wall is for the boss fight, in a room to the left on the table.

Level 3
In the Castle ruins at the start of the level, when you drop down through the broken floor, its on a bookshelf on the back wall.

Level 4
At the very end of the Level, before exiting, to the right of the stone it is sitting in one of the window-frames of the house, slightly hidden by an awning.

Level 5
When you first enter the Castle grounds, at the big tree, turn to right and go to the end of the street, then turn left, and duck inside the building. Take the lift upstairs, and jump across to the white ledge in front of you. Drop down and use the lever to open the gate. The book is inside the internal perimeter where you dropped down, hidden by some barrels.
