I found this monster on the roof of the second stage of the 2nd bonus level. (I had hard mode and silver death turned on)

Этого монстра я нашел на крыше второго этапа 2го бонусного уровня. (у меня был включен сложный режим и серебрянная смерть)

After you walk past this monster, there will be a characteristic sound, and the next time you start the game, there will be a new logo

После того как пройти мимо этого монстра будет характерный звук, а при следующем запуск игры будет новое лого

I got it too. THe music muted for the rest of the stage, then things went back to normal. I tried to access it again, but they place extra blocks that seal off that route. Also got the creepy title screen.
I'm guessing that's it for now.

I saw this too. I'm hoping to see it again to try and kill it with my bow. My screen did change. It plays some creepy violin and the screen glitches constantly. This is the android version. Too bad I didn't catch a screenshot of it. I saw this thing in normal mode.

I got the logo change after I rebooted after seeing it. Pretty interesting. I didn't get any achievements for it though

я его нашел на обычной сложности 2 ур бонусного уровня.. попасть туда можно тока на 2 этапе скорости. на 1 этапе. не запрыгнуть на дом. сам его увидел сегодня случайно..чуть не наложил в штаны.
