Mars Alpha pattern Leman Russ tank from WH40K Inquisitor Martyr game. Includes two variations, the Demolisher and Eradicator.

since I see no invitation link for Joazz's server and you insist to ask about the model, https://steamproxy.com/id/ninjabortnite you can go to ask ninja, the who ported the model, for if he'd like to share you. Personally I'd say that is a very rare percent if he consents to do so. Although I extremely don't want to post his Steam profile for these questions cuz I don't want him be bothered by some random role play server guys or model seekers who is not his acquaintance(this happens before and they stole the models for server using)
