How to get it?

This is so fast and simple.
1. Start a New Game, die once.
2. Complete first level.
3. Then go to Menu -> "Select Scene" -> Select "Scene 19".
4. Complete it to get last achievements.

And that's all!

lvl 19 is easy when u use the tunnel left from start in the ground.
