1. Fear Orcs: you get +5% bonus xp but when you fight orcs you do 33% less damage
2. minor frost resist (for minions) +10% frost resist +20 frost resist

Night Blindness: "Bad sight when it's dark", +2% Combat Experience
Claustrophobia: -17% attack and defense, +5% Combat Experience

Demon Killer: +50% damage to Demons (It's got a picture of a screen on it)
Orc Killer: +50% damage to Orcs
Imp Killer: +50% damage to Imps
Skeleton Killer: +50% damage to Skeletons

Lightning Damage: +25% lightning damage
Claws: +25% physical damage, can't use gloves
Deadly Damage: Increases skill damage 40%

Guardian Angel: +200% Good Avatar appearance.
Strong Will: +10 spirit, +10% spirit
Stalker Feet: Passive. Blocks Item Type: Boot. +25% Movement
Stunning Blow: Support/Passive. +40% Crushing Blow
Critical Hits: Support/Passive +40% Critical Hits
Cold Damage: Support/Passive +25% Cold Damage
Curse of Enfeeblement: Support/Passive. 25% to use on Kill. All Enemies in Range up to 6ft, deal -13% Physical Damage for 20 seconds. +23 Maximum Mana

Knockback: Support/Passive. 50% Knockback
Curse of Pain: Support/Passive. 25% Chance to use on Kill. +10% Attack Time, 3dmg every 3 seconds, for 20 seconds. +10 Maximum Mana.

Ya, it would be easiest to just look in the file that treborx555 mentioned.

I wanted to mention a couple cool ones though:
Mana Shield: All damage goes through your mana first
Makes for some incredibly tough characters as it is easier to boost mana than health, though you can't cast spells anymore once you get to health only.

Pain Delay: All incoming damage is converted into damage over time, however you take 20% more damage
This pretty much gets rid of instant deaths from crits because the crit damages over time and you have time to heal to counter it.

Otherwise, most of the basic skills seem to have a mutation version, though are usually less effective than the actual skill.

Claustrophobic... -15% attack and defense but +5% combat xp...lawls..got it at the same time I died to a cave in. Go figs.

I have found the following:

Stalker Feet: Passive. Blocks Item Type: Boot. +25% Movement

I got an awesome on that is like a Head hunter belt from Poe. You get mods from enemies for 1min but 75% less hp regen. you can become as big as your visible screen space , lol, with 1000+% hp modifiers. so much fun but not being able to regen you will need healing spells.

I feel like listing all these will be an ambitious project. :)
Here's the ones I currently have:

I've gotten other actual skills, but only when they're attached as support skills, so I suspect they aren't actual "mutations"
