Introduction
This is a Guide for the secret achievements in One Deck Dungeon. They are in order of difficulty in my opinion. It's a Work in Progress so feel free to leave a comment with some advice for anyone who visits this guide. I'll update when I can.
Riders on the Swarm
Face a swarm effect with 4 open doors, cover the wide box and survive!
Open all 4 doors while fleeing. Hopefully one of the encounters is either a Goblin or a Plague Rat. The armor box should say x[16]. Fight it and, as long as you cover the x[16] armor box and survive, the achievement will unlock.
The Better Part of Valor
Descend with 4 open doors.
Open all 4 doors. Beat an encounter and explore again for more doors, effectively eating up time. Use Archer's Heroic Feat to speed it up even more. Feel free to take some deliberate consequences in the form of time, leaving 6-8 cards in the deck. Pretty easy if timed well.

Can also be done if you have no doors and exactly 14 cards left. Explore then open and flee from all 4 doors. It would be exacly 14 cards and you can descend and get the achievement.
Flame Out
Lose a game by taking damage from the Flames effect.
You need to have 0 potions and a hero 1 hp away from death before fighting the Fire Elemental. Choose said hero and lose. Achievement unlocked!
Circling the Drain
Lose a game by converting an item with health due to the Drain effect.
Might take some time. You need to have 0 potions and have a health item. Be 1 hp away from death before fighting the Wraith. Convert the health item to XP and game over man. But hey you got the achievement!
You Got Gud
Cover every challenge box in a combat encounter after rolling too poorly to cover any one box.

DLC - Forest of Shadows

"Better not touch" - Lose a game by taking damage from running out of poison tokens.

"Trick or Treat" - Lose a game by converting poison due to the poison snapp effect.


DLC - Abyssal Depths

"Hold the Door" - Lose a game by Descending and taking damage from the Sharkwisp.

"Turtles all the way down" - Lose a game by gaining loot and taking damage from Terrorshell.

"Unfinished Business " - Lose a game by exploring with an open door and taking damage from the Sharkwisp.

Regarding "Circling the Drain":
Beware that you have to drink up all your potions before going down to 1HP and entering this combat encounter or the game will force you to drink one to survive. So frustrating, because leveling up puts you a step back.

You Got Gud - This is best done when rolling low low low (pure chance). There are two skills in the game that combine to amazing effect; chaotic aura (pick up to 5 die of a single number to change to 6s) and consistency (change any number of die to 4s). If you combine these two, you can effectively turn an entire roll of dice into 6s.
If you combine these two with a very unlucky roll, you can roll only (for example) 1s and 2s on a challenge that requires all 3s, 4s, 5s, or 6s to cover (or a larger box with a combined total higher than all of your dice). Then, use your skills to overcome.
Huge element of luck to this one, but setting up the right skills will make it happen.

Your senses tell you to stop, but you want it too much! Lose a game by taking damage from running out of poison tokens.

Wow, I would never have guessed that there are achievements tied to losing a game, much less through special effects. Thanks for the guide and thanks for the explanatory comments!

I wonder if Better not Touch is something like "Die by failing a resist poison roll" or "Survive with enough poison to kill you" or something. Now that I know that there are achievements tied to losing, this seems obvious.

Trick or treat you get similar to Flame out and Circling the drain, but against Trickster.

So I realize only 1 in 250 players have ever gotten there but any ideas about Better not Touch?

I completed a secret achievement called "Min Maxer" which is unlocked by choosing to exile every die of a color. I did this while fighting the Mud monster and chose to exile all of the pink dice.

To get You Got God, try running Calliana + mage or paladin. These gives you 6-7 blue dice and 1 red or 2 yellow.

Pick dragon for mage, hydra for paladin - so the first floor box is opposite of your color.

Then go find a bandit or rat pack - they have no blue challenge boxes. Pray that your 1-2 not blue dice roll 1s and 2s.

Then use whatever you can to switch dice around. Passive ability for blue -> yellow/red, calliana's ability, ability to get a free 3 pip black die, any potions/skills that you picked up earlier in the run.

Easy mistakes to make - Don't go to level 2. The black die is a liability. Don't take an item that will cover your third color, similarly.

I personally did it with paladin. Found a rat swarm and rolled 1 and 2 on my pair of yellows.
