"Better not touch" - Lose a game by taking damage from running out of poison tokens.
"Trick or Treat" - Lose a game by converting poison due to the poison snapp effect.
DLC - Abyssal Depths
"Hold the Door" - Lose a game by Descending and taking damage from the Sharkwisp.
"Turtles all the way down" - Lose a game by gaining loot and taking damage from Terrorshell.
"Unfinished Business " - Lose a game by exploring with an open door and taking damage from the Sharkwisp.
Beware that you have to drink up all your potions before going down to 1HP and entering this combat encounter or the game will force you to drink one to survive. So frustrating, because leveling up puts you a step back.
If you combine these two with a very unlucky roll, you can roll only (for example) 1s and 2s on a challenge that requires all 3s, 4s, 5s, or 6s to cover (or a larger box with a combined total higher than all of your dice). Then, use your skills to overcome.
Huge element of luck to this one, but setting up the right skills will make it happen.

I wonder if Better not Touch is something like "Die by failing a resist poison roll" or "Survive with enough poison to kill you" or something. Now that I know that there are achievements tied to losing, this seems obvious.
Pick dragon for mage, hydra for paladin - so the first floor box is opposite of your color.
Then go find a bandit or rat pack - they have no blue challenge boxes. Pray that your 1-2 not blue dice roll 1s and 2s.
Then use whatever you can to switch dice around. Passive ability for blue -> yellow/red, calliana's ability, ability to get a free 3 pip black die, any potions/skills that you picked up earlier in the run.
Easy mistakes to make - Don't go to level 2. The black die is a liability. Don't take an item that will cover your third color, similarly.
I personally did it with paladin. Found a rat swarm and rolled 1 and 2 on my pair of yellows.
