can you check a couple things? it would really help narrow down the issue
first add an item and check that it is added to the list (by scrolling down to the bottom of the config menu below the Reset option) if it's not there then it's not properly updating the list
second open the save.dat file at Steam\steamapps\common\The Binding of Isaac Rebirth\data\itembanlist it should look something like 1:A>B.:C>D. and you should see the ID numbers of the items, if not then the mod isn't saving its data
then quit game and reload, continue the run, open the config menu and scroll to the bottom again, if the list is empty open the save file again, if the items don't appear in the config menu but are still in the save file then the data isn't being loaded, if the items were also missing from the save then something is causing the mod to think the save file is corrupt and its resetting itself
are there any error messages in your debug console?
