I have been eaten by the spider through the secret hole under the church, at least 3 times, and the achievement (Persistence) has still not dropped. I have gotten other achievements with no issues - any ideas what I'm doing wrong?

Was looking at the achievements (and trophies on PS4 version). While the achievements entirely consisting of "How many different ways can you die" is pure dark humor, I feel that on top of them there should also be achievements/trophies for actually beating each game, with a gold trophy for each for the PS4 release and perhaps platinum for getting all trophies.
