I played the game with a different account and some achievements are bugged. I finished the game with 3 humans and i didnt get it - as well as i am unable to get the templar. What can i do?

Is It just randomly generated? Is there anyway to get different quests or quests in easier dungeons other than restarting or does the game only give new quests once an old one is completed?
